Eyelid surgery cost: upper lids, lower lids, or both

Upper and lower blepharoplasty each have their own code; done together, the room and anesthesia lines shrink.

Benchmark, not a quote: the operating room and anesthesia lines are 2026 Medicare rates for the same codes; private prices for cosmetic work are set by each practice and are often higher. No public file prices the surgeon's fee: it counts only once you enter the figure from your own quote.

Upper eyelid surgery comes to a room and anesthesia benchmark of $1,276 at national rates in 2026 and lower eyelid surgery to $1,280, before the surgeon's fee. Medicare pays a surgery center the same rate for both codes, CPT 15822 for the upper lids and 15820 for the lower, $1,129, and a hospital outpatient department $2,108. Anesthesia is short: about 33 minutes in the CMS file for the upper lids, $148 at the 2026 national factor. Both procedures in one session come to $1,890 for the room and anesthesia, the lower lids' room paid at half rate. The surgeon's fee is set by each practice and no public file publishes it, so it enters the calculator only from your quote. MedlinePlus describes the operation as done in a surgeon's office or a surgery center with numbing medicine injected around the eye, in which case a separate anesthesia bill may not exist.

Two codes, one rate

CMS gives the two operations the same facility rate and almost the same time, 33 minutes for the upper lids and 36 for the lower. On the lines public files price, the two cost nearly the same; whatever separates two quotes is the surgeon's fee and the facility's private prices. The MedlinePlus page on eyelid lift describes the operation as removing loose skin and extra fat through small cuts in the natural creases, and adds that it does not remove wrinkles around the eyes, lift sagging eyebrows or get rid of dark circles, which is worth knowing before comparing it with a brow lift.

When it is about vision

MedlinePlus explains that an eyelid lift is needed when drooping lids reduce vision, and that an eye doctor may test the visual field before surgery; done to improve appearance, it is cosmetic surgery. Health plans and Medicare follow the same line. When the upper lids block vision and the plan approves the operation, it becomes a covered procedure, and the patient pays the plan's usual share instead of this bill. The insurance page explains the approval process.

What readers ask about this bill

How much does eyelid surgery cost in 2026?

For the room and anesthesia at national rates, about $1,276 for the upper lids, $1,280 for the lower lids and $1,890 for both in one session, with a surgery center room of $1,129 at Medicare's rate for either code. The surgeon's fee comes on top; only your quote gives it.

Is lower eyelid surgery more expensive than upper?

Not on the lines public files price. Medicare pays the same facility rate for the two codes and records nearly the same time, so the room and anesthesia barely differ: $1,280 for the lower lids against $1,276 for the upper. Any gap between two quotes comes from the surgeon's own fee.

Will insurance pay for eyelid surgery?

Only when it is medically necessary, typically when drooping upper lids reduce vision and an eye examination documents it, and the plan approves the operation in advance. Eyelid surgery done to look younger or less tired is cosmetic, and plans and Medicare exclude it. Ask the plan for its written criteria before the visual field test.

These figures are benchmarks: public Medicare rates for the operating room and anesthesia of the same codes; the surgeon's fee, which no public file publishes, counts only from your own quote. A practice sets its own prices, often above Medicare rates, and only a surgeon who has examined you can say what you need. The site describes bills, never whether a procedure suits you, and earns nothing from any surgeon, clinic, drug maker or lender.
